Are you connected to our Patient Portal?
Two ways to connect:
On Computer/Laptop:
Ask the practice if you are web-enabled.
Once you are web-enabled, check your emails to set up your account. It will look like the following:
Click “Set up Portal Account.” You will be directed to send a code by receiving it via text or voice message on the phone associated with your account.
Once you receive the code, enter it and verify.
Create your password and set up your security questions.
Sign the consent form t complete your patient profile.
Your patient portal will look like this:
On Healow App (Smart Phone):
STEP 1
Download and install the Healow App on your mobile device. Open app and click on "Get Started"
STEP 2
Enter the Practice Code: EACABD
STEP 3
Enter your username and password
STEP 4
Choose and Select the relationship to the patient and click "Login"
